Job Description

We are looking to recruit several BANK Healthcare Support Workers within Social Care for multiple Community based roles in the Stoke on Trent area.

You will work closely with the Child/Adult and their family to deliver a tailormade package of support. Whilst placing the individual at the heart of the care package, you will ensure that their health and social care needs are met, empower them to achieve their goals and outcomes, whilst providing ongoing support with daily living activities.
